Application of graded pain management in patients after pancreatic surgery
分级化疼痛护理在胰腺术后患者中的应用

摘要

目的 分析在胰腺术后患者护理管理中应用分级化疼痛护理方案的临床效果。方法 将2024年8月-2026年2月于本院行胰腺手术的180例急性胰腺炎患者为研究对象，采用随机数字表法分为两组，即对照组和研究组各90例，分别予常规术后疼痛护理和分级化疼痛护理，比较两组患者术后不同时段疼痛程度（数字评分法，Numerical Rating Scale, NRS）变化、镇痛满意度和生活质量（急性胰腺炎患者报告结局量表，Patient‑reported Outcome Scale in Acute Pancreatitis, PAN-PROMISE）。结果 研究组术后不同阶段的NRS评分均低于对照组（P<0.05）；研究组镇痛满意率较对照组明显提升（P<0.05）；研究组PAN-PROMISE各维度评分较干预前有大幅提高，且比对照组更高（P<0.05）。结论 分级化疼痛护理方案可有效降低胰腺术后患者的疼痛程度，提升镇痛满意度，改善患者术后生活质量，是一种切实可行的术后疼痛管理模式，值得在临床推广应用。

Abstract

Objective To analyze the clinical effect of applying the graded pain management scheme in the nursing management of patients after pancreatic surgery.
Methods A total of 180 patients with acute pancreatitis who underwent pancreatic surgery in our hospital from August 2024 to February 2026 were selected as the research subjects. They were divided into two groups using the random number table method, namely the control group and the study group, with 90 cases in each group. The control group received conventional postoperative pain management, while the study group received graded pain management. The changes in pain degree (Numerical Rating Scale, NRS) at different postoperative time points, pain satisfaction, and quality of life (Patient-reported Outcome Scale in Acute Pancreatitis, PAN-PROMISE, an international prospective cohort study) were compared between the two groups.
Results The NRS scores of the study group at different postoperative stages were lower than those of the control group (P < 0.05); the pain satisfaction rate of the study group was significantly higher than that of the control group (P < 0.05); the scores of each dimension of PAN-PROMISE in the study group were significantly higher than those before intervention and were also higher than those of the control group (P < 0.05).
Conclusion   The graded pain management scheme can effectively reduce the pain degree of patients after pancreatic surgery, improve pain satisfaction, and improve the quality of life of patients after surgery. It is a feasible postoperative pain management model and is worthy of clinical promotion and application.
