摘要
目的 以患者旅程视角为核心，构建维持性血液透析（MHD）患者高磷血症护理管理模式，并验证该模式的临床应用成效。方法 选取2025年1月-2026年1月我院收治的100例MHD高磷血症患者，随机分为两组各50例。对照组采用常规护理，观察组应用基于患者旅程的专属护理模式。对比两组血磷代谢指标、自我管理能力及并发症发生情况。结果 干预后，观察组血磷水平、并发症发生率均显著低于对照组，自我管理能力及护理依从性显著高于对照组（P＜0.05）。结论 基于患者旅程构建的护理模式，能有效改善MHD高磷血症患者的血磷控制效果，提升自我管理能力，降低并发症风险，临床应用价值显著。

Abstract
Objective From the perspective of the patient journey, construct a nursing management model for maintaining hemodialysis (MHD) patients with hyperphosphatemia, and verify the clinical application effectiveness of this model. Methods 100 MHD patients with hyperphosphatemia admitted to our hospital from January 2025 to January 2026 were selected and randomly divided into two groups, with 50 cases in each group. The control group received routine care, while the observation group applied the exclusive nursing model based on the patient journey. The blood phosphorus metabolism indicators, self-management ability, and complication occurrence were compared between the two groups. Results After the intervention, the blood phosphorus levels and complication rates in the observation group were significantly lower than those in the control group, and the self-management ability and nursing compliance in the observation group were significantly higher than those in the control group (P < 0.05). Conclusion The nursing model constructed based on the patient journey can effectively improve the blood phosphorus control effect of MHD patients with hyperphosphatemia, enhance self-management ability, and reduce the risk of complications. The clinical application value is significant.
