The application of SpyGlass in elderly patients undergoing ERCP and perioperative nursing strategies
SpyGlass在老年ERCP患者中的应用与围术期护理策略

摘要

目的 探讨老年患者ERCP（Endoscopic Retrograde Cholangiopancreato-graphy）诊疗中使用SpyGlass经内镜直视系统的应用价值，以及围手术期护理要点，为老年ERCP患者这一高风险群体构建一道贯穿术前、术中、术后的安全防护网。方法 回顾性分析我院2025年1月至12月收治的15例接受SpyGlass辅助ERCP的老年(≥65岁)患者的临床资料，重点阐述术前准备、术中配合、术后观察及并发症预防等方面的护理措施。结果 15例患者均成功完成 SpyGlass辅助下的诊断或治疗，诊断准确率达[100]%，治疗成功率达[100]%。通过系统化的护理干预，患者术中生命体征平稳，术后未发生出血、感染、穿孔并发症，仅一患者血尿淀粉酶偏高，并发症发生率低于传统ERCP。结论 Spyglass内镜直视系统为老年胆胰疾病患者提供了更精准、微创的诊疗手段。而全面、精细化、个体化的围手术期护理是保障手术安全的关键。

Abstract

Objective To explore the application value of the SpyGlass endoscopic direct visualization system in ERCP diagnosis and treatment for elderly patients, as well as key perioperative nursing measures, to establish a comprehensive safety protection network for this high-risk group of elderly ERCP patients spanning preoperative, intraoperative, and postoperative phases.
Methods A retrospective analysis was conducted on clinical data of 15 elderly patients (≥65 years) undergoing SpyGlass-assisted ERCP admitted to our hospital from January to December 2025. The study focused on nursing interventions including preoperative preparation, intraoperative cooperation, postoperative monitoring, and complication prevention.
Results All 15 patients successfully completed diagnostic or therapeutic procedures under SpyGlass guidance, with diagnostic accuracy reaching [100]% and treatment success rate achieving [100]%. Through systematic nursing interventions, patients maintained stable vital signs during surgery, with no postoperative complications such as hemorrhage, infection, or perforation observed. Only one patient exhibited elevated serum amylase levels, demonstrating a lower complication rate compared to conventional ERCP.
Conclusion   The SpyGlass endoscopic direct visualization system provides more precise and minimally invasive diagnostic and therapeutic approaches for elderly patients with biliary and pancreatic diseases. Comprehensive, refined, and individualized perioperative nursing care remains critical for ensuring surgical safety.
