The impact of symptom cluster management-based nursing intervention on sleep quality of patients with lower limb fractures
基于症状群管理的护理干预对下肢骨折患者睡眠质量的影响

摘要

目的 深入且细致的分析探讨基于症状群管理的护理干预对下肢骨折患者睡眠质量的影响，旨在为临床研究提供理论与实践参照。方法 设立2023年9月至2024年9月为研究开展期限，纳入本时段院内收治的100例下肢骨折患者作为分析对象，遵循“平行序贯”分组原则，采用双色球分组法完成组间安排，每组归入50例（n=50）。参照组执行常规护理服务方案，研究组在开展常规护理的同时，增加基于症状群管理的护理干预，最终收集整理临床资料和相关数据，以睡眠质量、疼痛评分、焦虑评分、下肢肿胀程度、并发症发生率为观察指标，比对不同护理管理方案下的效果和差异。结果 研究组患者各项指标评分均显优参照组，P＜0.05。匹兹堡睡眠质量指数（Pittsburgh Sleep Quality Index,PSQI）评分更低，疼痛、焦虑、肢体肿胀程度的改善更明显，并发症发生率更低，差异具有统计学意义（P＜0.05），值得进一步探讨。结论 对于下肢骨折患者而言，在其临床干预策略的横向选择中，建议首选联合干预模式，即常规护理+基于症状群管理的护理，不仅能切实改善患者负面情绪及疼痛程度，同时还可有效减轻肿胀，提高患者睡眠质量，其应用价值显著，值得推广。

Abstract

Objective To conduct an in-depth and detailed analysis of the impact of symptom cluster management-based nursing interventions on sleep quality in patients with lower limb fractures, aiming to provide theoretical and practical references for clinical research.
Methods The study period was set from September 2023 to September 2024, with 100 hospitalized patients with lower limb fractures enrolled as the study subjects. Following the "parallel sequential" grouping principle, a two-color ball grouping method was employed to arrange the groups, with 50 cases assigned to each group (n=50). The control group received conventional nursing services, while the study group received additional symptom cluster management-based nursing interventions alongside routine care. Clinical data and relevant metrics were collected, with sleep quality, pain scores, anxiety scores, lower limb swelling severity, and complication incidence rates as observed indicators to compare the efficacy and differences between the two nursing management approaches.
Results All indicators in the study group showed significantly better scores compared to the control group (P<0.05). The PSQI score was lower, with more pronounced improvements in pain, anxiety, and limb swelling severity, along with a reduced complication incidence rate, all demonstrating statistically significant differences (P<0.05), warranting further investigation.
Conclusion   For patients with lower limb fractures, a combined intervention model—conventional nursing combined with symptom cluster management-based nursing—is recommended as the preferred clinical strategy. This approach not only effectively alleviates negative emotions and pain levels but also significantly reduces swelling and improves sleep quality, demonstrating substantial clinical value and potential for widespread adoption.
