One-needle subcutaneous tunneling technique in PICC catheterization and nursing intervention
一针式皮下隧道技术在PICC置管中的应用及护理干预

摘要

目的 探讨一针式皮下隧道技术在经外周静脉置入中心静脉导管（PICC）置管中的应用效果，配合系统化护理干预，观察其对置管成功率、并发症发生率、导管留置时间及患者舒适度的影响，为临床静脉通路管理提供安全高效的技术与护理方案。方法 选取2025年1月-2026年2月我院需行PICC置管患者50例，采用随机数字表法分为观察组和对照组，各25例，对照组采用常规超声引导下PICC置管+常规护理，观察组采用一针式皮下隧道技术置管，联合术前评估、术中配合、术后专项维护与并发症防控的系统化护理干预。比较两组穿刺口渗血、导管移位/脱出、静脉炎、导管相关感染、血栓发生率，以及留置时间、疼痛评分与护理满意度。结果 观察组各项并发症发生率及总并发症发生率均显著低于对照组（P＜0.05）；导管平均留置时间更长，置管疼痛评分更低，护理满意度更高（P＜0.05）。结论 一针式皮下隧道技术应用于PICC置管，配合规范化护理干预，可显著提升置管安全性与成功率，减少渗血、感染、血栓及导管移位等并发症，延长导管使用寿命，提高患者舒适度与满意度，值得在长期静脉治疗患者中推广应用。

Abstract

Objective To explore the application effect of one-needle subcutaneous tunneling technique in peripheral venous catheterization for central venous catheter (PICC), combined with systematic nursing intervention, and observe its influence on the success rate of catheterization, the incidence of complications, the duration of catheter retention, and the comfort of patients, in order to provide safe and efficient technical and nursing solutions for clinical venous access management.
Methods 50 patients requiring PICC catheterization in our hospital from January 2025 to February 2026 were selected and divided into the observation group and the control group by random number table method, with 25 cases in each group. The control group received conventional ultrasound-guided PICC catheterization+routine nursing, while the observation group received one-needle subcutaneous tunneling technique for catheterization, combined with preoperative assessment, intraoperative cooperation, postoperative specialized maintenance and complication prevention and control systematic nursing intervention. The incidences of puncture wound bleeding, catheter displacement/outcome, phlebitis, catheter-related infection, and thrombosis, as well as the retention time, pain score and nursing satisfaction were compared between the two groups.
Results The incidences of various complications and the total complication rate in the observation group were significantly lower than those in the control group (P < 0.05); the average catheter retention time was longer, the pain score of catheterization was lower, and the nursing satisfaction was higher (P < 0.05);
Conclusion   The one-needle subcutaneous tunneling technique applied in PICC catheterization, combined with standardized nursing intervention, can significantly improve the safety and success rate of catheterization, reduce complications such as bleeding, infection, thrombosis and catheter displacement, prolong the catheter retention time, improve patient comfort and satisfaction, and is worthy of promotion and application in long-term venous treatment patients.
