摘要

目的 总结1例婴幼儿发育迟滞伴右侧肢体运动障碍行高压氧治疗的精细化护理经验。方法 对1例9月龄患儿在常规康复基础上辅以共31次高压氧治疗。护理核心在于:治疗前进行全面评估与个性化准备;治疗中实施动态、预见性监护,精准管理压力变化与氧合状态;治疗后注重即时效果评价与系统性居家康复指导。结果 经综合干预后,患儿右侧肢体肌张力显著改善(Ashworth评分由Ⅲ级降至Ⅰ级),主动抓握潜伏期缩短至3~5 s,扶站右侧下肢支撑时间延长至15~20 s。治疗全程未发生中耳损伤、氧中毒等不良事件,家长焦虑情绪明显缓解(SAS评分由58分降至36分),并掌握了核心护理与康复要点。结论 针对婴幼儿高压氧治疗,实施以安全为核心、以家庭为协作者的精细化护理模式,是保障治疗安全性与有效性的关键。

Abstract

Objective To summarize the refined nursing experience for an infant with developmental delay and right-sided motor disorder undergoing hyperbaric oxygen therapy (HBOT).
Methods A total of 31 HBOT sessions were administered to a 9-month-old patient. The core nursing strategy included: comprehensive assessment and individualized preparation before treatment; dynamic and predictive monitoring during treatment, with precise management of pressure changes and oxygenation status; and immediate effect evaluation coupled with systematic home-based rehabilitation guidance after treatment.
Results After the treatment course, the child's muscle tone in the right limbs improved significantly (Ashworth scale score decreased from grade III to grade I). The latency for active grasping shortened to 3-5 s, and the right lower limb support time during assisted standing prolonged to 15-20 s. No adverse events, such as middle ear barotrauma or oxygen toxicity, occurred throughout the treatment. The parents' anxiety was significantly relieved (SAS score decreased from 58 to 36), and they mastered the key points of care and rehabilitation.
Conclusion   Implementing a safety-centric, family-collaborative, and refined nursing model is crucial for ensuring the safety and efficacy of HBOT in infants.
