Study on prone position ventilation nursing in elderly patients with severe pneumonia complicated with respiratory failure
俯卧位通气护理在老年重症肺炎合并呼吸衰竭中的研究

摘要

目的 观察分析对老年重症肺炎合并呼吸衰竭患者实施俯卧位通气护理的临床效果和应用价值。方法 选择院内呼吸与危重症医学科就诊的60例老年重症肺炎合并呼吸衰竭患者（样本纳入例数：60例；样本纳入时间：2023年5月至2024年5月），以随机数字为基础分组方式，分为对照、实验两个小组，观察例数高度一致。分别实施常规护理，俯卧位通气护理。收集分析护理效果。结果 与对照组（常规护理）相比，实验组（俯卧位通气护理）血氧分压、血氧饱和度更高，血二氧化碳分压更低，平均值动脉压更高，中心静脉压、心率更低，生活质量评分（角色功能/情绪功能/认知功能/社会功能/躯体功能/总体健康）更高（P<0.05）。结论 俯卧位通气护理可以有效促进老年重症肺炎合并呼吸衰竭患者症状改善，控制病情促进恢复，改善生活质量，有较高应用价值。

Abstract

Objective To observe and analyze the clinical effect and application value of prone position ventilation nursing for elderly patients with severe pneumonia complicated with respiratory failure.
Methods 60 elderly patients with severe pneumonia combined with respiratory failure were selected from the Department of Respiratory and critical care medicine in our hospital (sample included: 60 cases; Sample inclusion time: May 2023 to May 2024) was divided into control and experimental groups based on random numbers, and the number of observed cases was highly consistent. Routine nursing and prone position ventilation nursing were carried out respectively. The nursing effect was collected and analyzed.
Results Compared with the control group (conventional care), the experimental group (prone ventilation care) had higher blood oxygen partial pressure, blood oxygen saturation, lower blood carbon dioxide partial pressure, higher mean arterial pressure, lower central venous pressure, lower heart rate, and higher quality of life score (role function/emotional function/cognitive function/social function/physical function/general health) (P<0.05).
Conclusion   Prone position ventilation nursing can effectively promote the improvement of symptoms in elderly patients with severe pneumonia complicated with respiratory failure, control the condition and promote recovery, improve the quality of life, and have high application value.
