Study on the application effect of case management model in the rehabilitation care of elderly patients with chronic diseases
个案管理模式在老年慢性病患者康复护理中的应用效果研究

摘要

目的 深入探究个案管理模式于老年慢性病患者康复护理中的应用成效。方法 择取2020年8月1日至2024年8月1日在我院接受治疗的122位老年慢性病患者，随机划分成观察组与对照组，各61例。对照组运用常规护理模式，观察组则采用个案管理模式。对两组患者的护理最终效果、生活质量以及总体满意程度予以比较。结果 观察组患者的护理成效显著优于对照组，生活质量评分明显高于对照组，满意度亦显著高于对照组，差异皆具统计学意义（P<0.05）。结论 个案管理模式在老年慢性病患者康复护理中具有突出的应用效果，可提升护理质量，改良患者的生活质量，提高患者满意程度，值得在临床上广泛推广应用。

Abstract

Objective To deeply investigate the effectiveness of the application of case management mode in the rehabilitation nursing of elderly patients with chronic diseases.
Methods 122 elderly patients with chronic diseases who were treated in our hospital from August 1, 2020 to August 1, 2024 were randomly divided into the observation group and the control group, each with 61 cases. The control group utilized the conventional nursing model, while the observation group adopted the case management model. The final effect of nursing care, quality of life and overall satisfaction of the two groups were compared.
Results The nursing outcomes of patients in the observation group were significantly better than those of the control group, and the quality of life scores were significantly higher than those of the control group, and the satisfaction level was also significantly higher than that of the control group, with the differences being statistically significant (P<0.05).
Conclusion   The case management model has outstanding application effects in the rehabilitation nursing of elderly patients with chronic diseases, which can improve the quality of nursing care, improve the quality of life of patients, and increase the degree of satisfaction of patients, and is worthy of being widely popularized and applied in clinical practice.
