Application of prone position ventilation in critically ill patients with indwelling artificial airway outside the mind
俯卧位通气在神外重症留置人工气道患者中的应用

摘要

目的 探讨俯卧位通气在神外重症留置人工气道患者中的效果。方法 选取 2022 年 3 月~2023 年 3 月我院神经外科重症监护病房内收治的 80 例神经外科留置人工气道的重症患者,采取随机数字表法将其分为对照 组及观察组，各 40 例。对照组使用仰卧位进行通气，观察组使用俯卧位进行通气，两周观察期。分析 结果 干 预前，两组患者血气各项指标无明显异常，（P>0.05）；干预后，相对于对照组，观察组患者血气各项指标均有 改善，情况明显好转，（P<0.05）；和对照组相比，观察组患者使用俯卧位通气后胸片好转率明显更高，（P<0.05）。 结论 应用俯卧位通气法治疗神外重症留置人工气道患者, 能够取得满意效果，该法可减少吸痰频次，有助于改 善患者血气分析指数，提升疾病治疗有效率，安全性强，减少患者经济负担，提高患者及家属满意度，值得临床 实践应用。

Abstract

Objective To explore the effect of prone position ventilation in patients with severe indwelling artificial airway outside the mind.
Methods 80 critical patients with artificial airway in neurosurgery intensive care unit of our hospital from March 2022 to March 2023 were selected. They were randomly divided into control group and observation group with 40 patients in each group. The control group was ventilated in a supine position, while the observation group was ventilated in a prone position for a two-week observation period. Analysis
Results Before intervention, there were no significant abnormalities in the blood gas indicators of the two groups of patients (P>0.05); After intervention, compared to the control group, all blood gas indicators of the observation group patients improved significantly (P<0.05); Compared with the control group, the observation group had a significantly higher improvement rate in chest X-ray after using prone position ventilation (P<0.05).
Conclusion   The application of prone position ventilation method in the treatment of patients with severe indwelling artificial airway outside the mind can achieve satisfactory results. This method can reduce the frequency of sputum aspiration, help improve the patient's blood gas analysis index, improve the disease treatment efficiency, have strong safety, reduce the patient's economic burden, and improve the satisfaction of patients and their families. It is worth clinical practice application.
