Analysis of the preventive effect of plantar pump combined with systematic nursing on patients with deepvenous thrombosis of lower limbs in operating room
足底泵联合系统护理对手术室患者下肢深静脉血栓的预防作用分析

摘要

目的 对足底泵联合系统护理对手术室患者下肢深静脉血栓的预防作用进行分析和探讨。方法 以我院 2020 年 2 月-2021 年 3 月期间收治的 126 例手术患者为对象展开随机对照研究，分组方法采用随机 数字表法，63 例纳入对照组，术中予以常规手术室护理干预；63 例纳入观察组，术中予以足底泵联合手术 室系统护理干预。对比两组患者手术情况、下肢深静脉血栓发生情况等指标。结果 两组患者术中体温、平 均动脉压、出血量以及手术时间等一般手术指标值相当(P>0.05)；观察组术后 1d 的 D-二聚体、纤维蛋白原 水平以及凝血酶原时间显著低于对照组(P<0.05)，股静脉、胫后静脉、腘静脉的血流速度显著高于对照组 (P<0.05)；观察组组内术后即刻、1d、7d 的下肢深静脉血栓发生率（分别为 1.59%、6.35%、9.52%）显著低 于对照组（分别为 7.94%、19.05%、36.51%）(P<0.05)。结论 术中护理时，足底泵联合系统护理措施的运 用不会影响患者手术的进行，同时能够有效改善患者凝血功能指标、优化术后下肢各静脉血流速度，是预 防手术室患者下肢深静脉血栓的有效措施，具备临床推广应用价值。

Abstract

Objective to analyze and explore the preventive effect of plantar pump combined with systematic nursing on deep venous thrombosis of lower limbs in patients in the operating room.
Methods a randomized controlled study was carried out on 126 surgical patients admitted to our hospital from February 2020 to March 2021. The grouping method was random number table method. 63 patients were included in the control group and were given routine nursing intervention in the operating room during the operation; 63 cases were included in the observation group, and were given plantar pump combined with systematic nursing intervention in the operating room during operation. The operation condition and the incidence of deep venous thrombosis of lower limbs were compared between the two groups.
Results the temperature, mean arterial pressure, blood loss and operation time of the two groups were similar (P > 0.05); The levels of D-dimer, fibrinogen and prothrombin time in the observation group were significantly lower than those in the control group (P < 0.05), and the blood flow velocity of femoral vein, posterior tibial vein and popliteal vein was significantly higher than that in the control group (P < 0.05); The incidence of DVT in the observation group was significantly lower than that in the control group (7.94%, 19.05% and 36.51% respectively) (P < 0.05).
Conclusion   during intraoperative nursing, the application of plantar pump combined with system nursing measures will not affect the operation of patients, but can effectively improve the coagulation function index of patients and optimize the blood flow velocity of lower limb veins after operation. It is an effective measure to prevent deep venous thrombosis of lower limbs in patients in the operating room, and has clinical promotion and application value.
